
Rural Hospitals Are Closing and Medicaid Cuts Are Being Blamed

In rural northeast Georgia, a hospital closed its maternity ward. In rural New Hampshire, a community health center shut down entirely. In Iowa, a major hospital system laid off dozens of employees and closed a clinic. These are not projections - they are events that already occurred in the months following the passage of the ‘Big Beautiful Bill’ in July 2025.
A new analysis published in April 2026 found that more than 400 hospitals across the United States are now at high risk of closing or cutting services as a direct result of the Medicaid cuts embedded in that legislation, according to NBC News.
The Scale of What Was Signed
President Trump signed the One Big Beautiful Bill Act into law in July 2025. Among its provisions were the largest cuts to Medicaid in the program’s history. The Congressional Budget Office estimated that roughly 10 million Americans would lose coverage as a result, with the full impact rolling out primarily in 2027 and 2028.
But hospitals say they cannot wait until the cuts take effect to start making decisions. Trinity Health, a Michigan-based hospital system, announced it expected to lose $1.5 billion due to policy changes and had already cut more than 10 percent of its billing staff. One of its facilities in rural Georgia closed its maternity unit. Alameda Health System in Oakland told NBC News that the cuts represent ‘an existential threat,’ projecting losses of more than $100 million annually by 2030.
Who Gets Hit Hardest
The at-risk hospitals identified in the analysis span 44 states and Washington, D.C. Roughly 60 percent are in urban areas - but experts say rural hospitals face the most severe consequences because they are often the only care option within a reasonable distance.
‘For a lot of communities, especially those in rural areas, hospitals may be the only place to get that care,’ James Jackson, CEO of Alameda Health System, told NBC News. ‘If we’re not able to provide a service, there will be a delay in receiving care at one of the other systems in the area - or they may not provide it at all.’
ABC News documented what that already looks like in Chippewa Falls, Wisconsin, where Sacred Heart Hospital shut down last year with little notice, terminating nearly 1,400 employees. First responders told ABC that the closure has made it harder to stabilize critical patients before transport. ‘It can be a loss of life,’ the local fire station chief said.
The Political Dimension
Democrats have moved quickly to make the hospital closures a defining issue in the 2026 midterm elections. The Democratic Congressional Campaign Committee has vowed to make Medicaid cuts ‘the defining contrast’ of the election cycle.
Some Republican incumbents have already felt the pressure. California Rep. David Valadao, who represents a district where more than 60 percent of residents rely on Medicaid, voted for the bill after previously warning he could not support it. A constituent who relies on Medicaid told NBC News he felt ‘betrayed.’ A California doctor has since launched a congressional campaign specifically citing the vote.
Sen. Joni Ernst of Iowa drew national attention after telling a town hall protester ‘well, we all are going to die’ in response to questions about Medicaid coverage - a comment that has since generated multiple Democratic challengers in her 2026 Senate race.
The Window Before It Gets Worse
Most of the Medicaid cut provisions do not take full effect until 2027 and 2028. But health experts say the hospital system is already reacting now. CNN reported in March 2026 that rural health clinics have begun closing, maternity wards are disappearing, and hospital administrators are making service decisions today that they cannot reverse once the cuts arrive.
The $50 billion rural relief fund written into the legislation is, by the administration’s own math, a fraction of the estimated $137 billion in federal Medicaid spending reductions projected for rural areas over the next decade. For communities where the hospital is the only ER, the only labor and delivery unit, and the only psychiatric bed within 50 miles, that gap is not abstract.
